The M/R-3004 MPI Enter Visit Data Screen is used to setup or maintain the Patient's Visit Information.


ADD:         To add new visits to the patients record.


Change Data: To maintenance visit information on the patients record.


Delete: To remove vistis from the patients record.


EXIT: To return and enter another patient or exit the data screen

Medical Record Number: this field is used to reflect the Patient's Medical Record Number.  If the Medical Record Number is unknown click the Search Button to search the MPI (Master Patient Index) for the medical record number.


Once a valid Medical Record Number has been entered the Patient's Fist, Middle, and Last Name will be displayed.


Once the Process Button is clicked the Add, Change, Delete, and New Patient Button will be displayed.  These buttons are available with this screen.  Select the appropriate action, by clicking on the button of your choice or by highlighting your choice and pressing the Enter key.


Admission Date: This field is used to identify the date of admission for an inpatient or the visit date of an outpatient.  The date format should be MM/DD/CCYY.


Visit Control:  If you are in the Add mode you will not be prompted to this field.  If you are in any other mode you must enter the visit number you are modifying, deleting, or inquiring about for this patient for a particular admission date.


Discharge Date: This field is used to reflect the Discharge Date if the patient was an inpatient.  If the patient was an outpatient it will reflect the date of visit. The date format should be MM/DD/CCYY.


Note: Care should be taken with this entry, as it is used to age most reports.


Doctor Number: This field is used to reflect the Admitting Doctor Number associated with the patient for this visit.


If the Doctor is unknown click the Search Button to view and select from a list of the available responses. Doctors are established within the Patient Accounting Application.


Service: This field is used to reflect the Service Class that is assigned to a patient for this visit.


If the Service is unknown click the Search Button to view and select from a list of the available responses.  Service Classes are established within the Patient Accounting Application.


ICD9CM Diagnosis Code: This field is used to reflect the ICD9CM (International Classification of Diseases, 9th Revision, Clinical Modifications) Diagnosis Code.  The ICD9CM code must be established within the system in order to be used.  This is an optional field and can be left blank.


ICD9CM Operative Code: This field is used to reflect the ICD9CM Operative Code.  The ICD9CM code must be established within the system in order to be used.  This is an optional field and can be left blank.


Patient Type: this field is used to reflect the Patient Type.


If the Patient Type is unknown click the Search Button to view and select from a list of the available responses.  Patient Types are established within Table 19 of the System Variables Application.


Discharge Status: This field is used to reflect the Status of the Patient at the time of discharge.


If the Discharge Status is unknown click the Search Button to view and select from a list of the available Table 4 of the System Variables Application. 


Discharge Disposition: This field is used to identify the Disposition of the Patient at the time of discharge.


If the Discharge Disposition is unknown click the Search Button to view and select from a list of the available responses. Dispositions are established within Table 7 of the System Variables Application.


UB Code: this field identifies the patient's discharge.


FSC Code: this field identifies the payment class for the patient.


Account Number: This field is used to identify the Patient's Account Number.


If you are in the DELETE mode and the patient displayed is the patient you wish to delete click on the Delete Button. The Visit Information Will Be Marked To Be Removed From The Database.
