X-4105 Doctor Index By Patient Listing (Selective)

The X-4105 Doctors Index Listing is produced through the Print Doctors Index option within the Medical Records Reporting menu.

This report may be obtained at any time and is generated by user defined criteria.

The report includes the date, time, and user who ran the option. The report is a double column report that displays the Doctor's Name, the Doctor's Number, the Medical Record Number, the Patient's Last Name, the Patient Type, Inpatient or Outpatient, the Doctor Type, the Discharge Date, the Service Class, and the Results.

Also included on the report is a total sheet that is generated from the Doctors Index Listing. The Total Sheet is used to summarize and display the subtotals and cumulative totals for the Patient Types, the Doctor Type Codes, and the Disposition Codes.
